Anybody who sells alcohol on a licensed premises must have authorisation from a personal licence holder. Use this template to keep a written record of who is authorised.

Under the Licensing Act 2003, a personal licence holder must authorise all staff who sell alcohol. This list should be stored somewhere it can be easily accessed in the event of an inspection by the police or licensing authorities. Copies should be kept on site.
